    A comprehensive model for x-ray projection imaging system efficiency and image quality characterization in the presence of scattered radiation.

    This work proposes a method for assessing the detective quantum efficiency (DQE) of radiographic imaging systems that include both the x-ray detector and the antiscatter device. Cascaded linear analysis of the antiscatter device efficiency (DQEASD) with the x-ray detector DQE is used to develop a metric of system efficiency (DQEsys); the new metric is then related to the existing system efficiency parameters of effective DQE (eDQE) and generalized DQE (gDQE). The effect of scatter on signal transfer was modelled through its point spread function (PSF), leading to an x-ray beam transfer function (BTF) that multiplies with the classical presampling modulation transfer function (MTF) to give the system MTF. Expressions are then derived for the influence of scattered radiation on signal-difference to noise ratio (SDNR) and contrast-detail (c-d) detectability. The DQEsys metric was tested using two digital mammography systems, for eight x-ray beams (four with and four without scatter), matched in terms of effective energy. The model was validated through measurements of contrast, SDNR and MTF for poly(methyl)methacrylate thicknesses covering the range of scatter fractions expected in mammography. The metric also successfully predicted changes in c-d detectability for different scatter conditions. Scatter fractions for the four beams with scatter were established with the beam stop method using an extrapolation function derived from the scatter PSF, and validated through Monte Carlo (MC) simulations. Low-frequency drop of the MTF from scatter was compared to both theory and MC calculations. DQEsys successfully quantified the influence of the grid on SDNR and accurately gave the break-even object thickness at which system efficiency was improved by the grid. The DQEsys metric is proposed as an extension of current detector characterization methods to include a performance evaluation in the presence of scattered radiation, with an antiscatter device in place

    In Which Situations Do Adolescents Seek Parental Support and What Do They Need?

    Throughout development, the conditions that elicit children’s support seeking behaviour change hand in hand with maturation and changing developmental tasks. Little is known about the situations in which adolescents need support or about which parental behaviours elicit adolescents’ sense of being supported. In the current study, adolescents living in the Netherlands (N = 98, Mage = 15.5, SD = 1.16, range = 14–18 years) filled in online questionnaires in which they were asked to recall autobiographical support-related memories (ASMs) separately for both parents. Support from both parents was most frequently needed in case of concerns about competence. Loss or illness of close others or pets elicited primarily a need for maternal support whereas family conflicts and practical problems most frequently elicited a need for paternal support. Additionally, parent behaviour reported in times of distress was most frequently related to comforting and facilitating support seeking. These behaivours were rated as satisfying and sufficiently supportive for the reported distress. These results confirm the importance of parental support in adolescence. Clinical implications of these findings and directions for future research are discussed

    Lack of trust in maternal support is associated with negative interpretations of ambiguous maternal behavior

    Attachment theory assumes that children who lack trust in maternal availability for support are more inclined to interpret maternal behavior in congruence with their expectation that mother will remain unavailable for support. To provide the first test of this assumption, early adolescents (9-13 years old) were asked to assess whether ambiguous interactions with mother should be interpreted in a positive or a negative way. In our sample (n = 322), results showed that early adolescents' lack of trust in their mother's availability for support was related to more negative interpretations of maternal behavior. The associations remained significant after controlling for depressive mood. The importance of these findings for our understanding of attachment theory, attachment stability, and clinical practice are discussed

    The response of tropical precipitation to Earth's precession: the role of energy fluxes and vertical stability

    The changes in Earth's precession have an impact on the tropical precipitation. This has been attributed to the changes in seasonal solar radiation at the top of the atmosphere. The primary mechanism that has been proposed is the change in thermal gradient between the two hemispheres. This may be adequate to understand the zonal mean changes, but cannot explain the variations between land and oceans. We have used a simple model of the intertropical convergence zone (ITCZ) to unravel how precipitation changes with precession. Our model attributes the changes in precipitation to the changes in energy fluxes and vertical stability. We include the horizontal advection terms in this model, which were neglected in the earlier studies. The final response of the land and oceans is a result of complex feedbacks triggered by the initial changes in the insolation. We find that the changes in precipitation over the land are mainly driven by changes in insolation, but over the oceans, precipitation changes on account of changes in surface fluxes and vertical stability. Hence insolation can be a trigger for changes in precipitation on orbital timescales, but surface energy and vertical stability play an important role too. The African monsoon intensifies during a precession minimum (higher summer insolation). This intensification is mainly due to the changes in vertical stability. The precipitation over the Bay of Bengal decreases for minimum precession. This is on account of a remote response to the enhanced convective heating to the west of the Bay of Bengal. This weakens the surface winds and thus leads to a decrease in the surface latent heat fluxes and hence the precipitation.</p

    From trust in caregivers' support to exploration : the role of openness to negative affect and self-regulation

    Attachment theory assumes that trust in caregivers' support and exploration are closely related. Little research tried to investigate this link, nor focuses on mechanisms that might explain this association. The present studies examined whether trust is related to exploration through a serial indirect effect of openness to negative affect and self-regulation. In Study 1, 212 children, aged 8-13, completed questionnaires assessing trust, openness to negative affect, self-regulation and exploration. The results showed that trust predicted exploration, but only to the extent to which openness to negative affect and self-regulation were involved too. Study 2 refined these findings (n = 59, aged 9-12) using a behavioral measure of openness to negative affect and exploration, and with mother-reported self-regulation. Replicating this serial indirect effect of openness to negative affect and self-regulation with multiple informants and methods, the present studies advance our understanding of how trust might foster exploration in preadolescence

    Dynamic contrast-enhanced and diffusion-weighted MRI for early detection of tumoral changes in single-dose and fractionated radiotherapy: evaluation in a rat rhabdomyosarcoma model

    We aimed to examine different intratumoral changes after single-dose and fractionated radiotherapy, using diffusion-weighted (DW) and dynamic contrast-enhanced (DCE) MRI in a rat rhabdomyosarcoma model. Four WAG/Rij rats with rhabdomyosarcomas in the flanks received single-dose radiotherapy of 8 Gy, and four others underwent fractionated radiotherapy (five times 3 Gy). In rats receiving single-dose radiotherapy, a significant perfusion decrease was found in the first 2 days post-treatment, with slow recuperation afterwards. No substantial diffusion changes could be seen; tumor growth delay was 12 days. The rats undergoing fractionated radiotherapy showed a similar perfusion decrease early after the treatment. However, a very strong increase in apparent diffusion coefficient occurred in the first 10 days; growth delay was 18 days. DW-MRI and DCE-MRI can be used to show early tumoral changes induced by radiotherapy. Single-dose and fractionated radiotherapy induce an immediate perfusion effect, while the latter induces more intratumoral necrosis

    Hysteresis of tropical forests in the 21st century

    Tropical forests modify the conditions they depend on through feedbacks at different spatial scales. These feedbacks shape the hysteresis (history-dependence) of tropical forests, thus controlling their resilience to deforestation and response to climate change. Here, we determine the emergent hysteresis from local-scale tipping points and regional-scale forest-rainfall feedbacks across the tropics under the recent climate and a severe climate-change scenario. By integrating remote sensing, a global hydrological model, and detailed atmospheric moisture tracking simulations, we find that forest-rainfall feedback expands the geographic range of possible forest distributions, especially in the Amazon. The Amazon forest could partially recover from complete deforestation, but may lose that resilience later this century. The Congo forest currently lacks resilience, but is predicted to gain it under climate change, whereas forests in Australasia are resilient under both current and future climates. Our results show how tropical forests shape their own distributions and create the climatic conditions that enable them

    Is It Time for a Change? A Cost-Effectiveness Analysis Comparing a Multidisciplinary Integrated Care Model for Residential Homes to Usual Care

    OBJECTIVE: The objective of this study was to evaluate the cost-effectiveness of a Multidisciplinary Integrated Care (MIC) model compared to Usual Care (UC) in Dutch residential homes. METHODS: The economic evaluation was conducted from a societal perspective alongside a 6 month, clustered, randomized controlled trial involving 10 Dutch residential homes. Outcome measures included a quality of care weighted sum score, functional health (COOP WONCA) and Quality Adjusted Life-Years (QALY). Missing cost and effect data were imputed using multiple imputation. Bootstrapping was used to analyze differences in costs and cost-effectiveness. RESULTS: The quality of care sum score in MIC was significantly higher than in UC. The other primary outcomes showed no significant differences between the MIC and UC. The costs of providing MIC were approximately €225 per patient. Total costs were €2,061 in the MIC group and €1,656 for the UC group (mean difference €405, 95% -13; 826). The probability that the MIC was cost-effective in comparison with UC was 0.95 or more for ceiling ratios larger than €129 regarding patient related quality of care. Cost-effectiveness planes showed that the MIC model was not cost-effective compared to UC for the other outcomes. INTERPRETATION: Clinical effect differences between the groups were small but quality of care was significantly improved in the MIC group. Short term costs for MIC were higher. Future studies should focus on longer term economic and clinical effects. TRIAL REGISTRATION: Controlled-Trials.com ISRCTN11076857
